LD flap with no implant (totally autologous technique) is possible in only 20% of patients for example those with a high BMI or those with very small breasts. This 60 year old had a right mastectomy and immediate reconstruction with an LD flap without an implant. A left balancing breast reduction was performed at the same time as the mastectomy and reconstruction (results after one year). She declined nipple reconstruction.
